Sorry to be tiresome, but I think there is a problem in replying to this survey. I have answered 'no specific moment' because I had no road to Damascus experience, and I think this is what lies behind your question? But I was christened at 3 months old, and I suppose made a commitment then - or it was made on my behalf. And I again made a commitment at confirmation aged 14, so perhaps that is what I should have answered. So should I vote three times with three answers?!
